Welcome to VFW of Wisconsin's online form section. If you simply need to download a form, click here and you will be taken to the form repository. You can select the type of form you need and follow the steps to download your PDF form. There is no need to register at this site or login to download a PDF form. The Department of Wisconsin is also pleased to announce that we are beginning a new program for online form submission. We are piloting the program with the Post Action Report form. To access this form, you must register and login to the site for security purposes. You may register with any username or password you like. 
After you submit your registration, you will be sent an email address to verify your account. Click on the verification link provided to verify your account. You may then login to the online forms area. After you have logged in, select "WI Online Forms" in the menu on the left.  Then follow the links to the online Post Action Report. Once there, simply follow the instructions for each question, print the form if you want a copy, then submit it.
